Essure Reversal/Essure Removal
Essure is a form of permanent sterilization where metal coils are placed partially into the fallopian tube. Scarring then forms over the coils leading to tubal blockage. Some doctors feel that the only solution for removal of the device in the face of an adverse effect is a hysterectomy. This is not the case. The Essure device can be removed from the fallopian tube via either a hysteroscopic or laparoscopic approach. Again, hysterectomy is NOT usually needed.
Why remove Essure??? Essure has been on the market since 2002; however, through May 2015, the FDA has received over 5,000 reports of adverse effects related to Essure. These include chronic pelvic pain, painful periods, constitutional symptoms (rashes, joint pain, hair loss, weight changes), and a small amount of deaths that may, or may not, be associated with the device. These effects are thought to be related to incorrect placement or migration of the device or allergies to the nickel that is located around the coils.
It is important to know that Essure "reversal" is not the same as Essure removal. Because of the mechanism of how Essure works (see below) it cannot be "reversed." What centers usually mean by reversal is a procedure where the Essure coil and surrounding tube are removed, and the healthy portion of the remaining tube is reconnected to the uterus. This is similar to a standard tubal reversal procedure, but significantly more difficult.
Because it is a more difficult procedure, studies on pregnancy rates show that only about one fourth of women will have a successful pregnancy after Essure “reversal.” Therefore, pregnancy is best achieved with IVF. Essure removal is indicated when planning IVF because their appears to be a significantly lower pregnancy rate with IVF if the Essure is left in place. The live birth rate with IVF is only 22.2% in the presence of Essure compared with 37.4% if the device is removed prior to IVF.
